• A List of Current Prescriptions & Non-Prescription Medications/ Supplements - (NOTE: Pictures of the physical pills are not helpful to us. We need the names of your medications)
• A List of Allergies to Medications/ Foods
• Any Recent Tests/ Diagnostic Imaging Related to Your Current Medical Concern
• Insurance Information - including copies of your policy. (NOTE: It is important for YOU to contact your insurance company prior to coming into the clinic). Most insurance companies will require you to pay out-of-pocket and apply for reimbursement. Don't worry, our clinic is committed to providing fair prices to our patients.
